If you look on ebay under Kustom then go to musical instruments you will see that someone has handles....new for sale $21.50 plus shipping. This is the complete handle. I ordered 4 so there is 6 still left at the time of this posting.

The handles on ebay are for the newer Kustom Coupe 36-72 amps. These handles are wider than the original kustom handles from the 60's. The original handles, the inner most holes on hinges measure 3 7/16 inches apart. The newer handles, the seller states the inner most holes on hinges measure 4 inches apart.

Thanks I saw that cabinet handle for $21.50 as well. Since my rig is a late 60's The one on E bay may not have fit correctly. I was going to measure the screw holes on mine, but I bet they will be less than 4 inches apart as you stated.

There was a supplier in Wisconsin that modified those handles to fit the original hole pattern..... no longer available today but the basic mod is a bit of bending the handle frames to move the holes into a usable size. I have 2 pair of these on some black Pa 4-12 cabs. Worked fine for me.
