I guess I should have posted this last year when I first worked on the same amp as I did again last night, but it looks like new Acutronics reverb pans are made with solid core wire unlike the stranded type they use to have that goes from the rca jack to the transducer and these snap off within a year or so of use!
This was in a fellows Orange rocker reverb amp.
Last year I cut off the slip on connectors they come with on the transducer end and soldered them dirrectly to the pins on the transducer, now this time the wire gave out at the jack end even though that end has a vibration/ strain releaf.
If you have one of these pans I would suggest you place a nice big blob of silicone sealer to hold the slip on connector to the transducer and each wire to the female rca end.
last night I wound up replacing all 4 wires with 22ga stranded type and then did the silicone thing to boot!
